Below you’ll find answers to the questions we hear most often about eligibility, getting started, and what to expect from Access Community Care.

If you have questions that aren’t answered here, contact us anytime. We’re always happy to help families find their footing.

How do I know if I’m eligible for services?

If you’ve been determined eligible for Office of Developmental Disabilities Services (ODDS) through your local Community Developmental Disabilities Program (CDDP) or Brokerage, you may also qualify for services through Access Community Care. If you’re not sure, don’t worry, we’re happy to answer your questions and point you in the right direction.

How do I get started?

Getting started is easy. Contact us directly or ask your Service Coordinator or Personal Agent to submit a referral. Once we receive it, we’ll schedule an intake meeting to learn more about your goals, answer your questions, and determine whether we’re the right fit for your support needs.

Can I choose my Direct Support Professional (DSP)?

Yes! We believe the right match matters. We carefully pair participants with DSPs based on personality, interests, location, schedule, and support needs. If the first match isn’t the right fit, we’ll continue working with you until we find one that is.

What services do you provide?

We offer individualized supports that help people live as independently and confidently as possible. Depending on your authorized services, supports may include:

  • Activities of Daily Living (ADL/IADL)
  • Community Living Supports
  • Community Inclusion
  • Day Support Activities (DSA)
  • Employment Services
  • Skills Training
  • Personal Care Assistance

Every support plan is tailored to each participant’s unique goals, preferences, and support needs.

What makes Access Community Care different?

At Access Community Care, we don’t believe in one-size-fits-all services. We take time to build relationships, listen to what matters most to each participant, and provide compassionate, person-centered support that promotes independence, dignity, and community inclusion. Our goal is to help every person build independence, achieve their goals, and thrive, not simply receive services.

Can my services change if my needs change?

Absolutely. As your goals and support needs change, we work closely with you and your Service Coordinator or Personal Agent to ensure your services continue to reflect your Individual Support Plan (ISP) and authorized supports.

How long does it take to begin services?

Every situation is different, but once we receive your referral and required paperwork, we’ll move through the intake process as quickly as possible. Our team will keep you informed every step of the way and work to connect you with services as soon as they’re available.

Do you serve my area?

Access Community Care provides services throughout the Portland metropolitan area and many surrounding communities. If you’re unsure whether we serve your location, just give us a call, we’re happy to help.

Do I have to choose Access Community Care?

No. Choosing a provider is your decision. We’ll take the time to answer your questions, learn about your goals, and help you determine whether we’re the right fit. If we’re not, we’ll do our best to point you in the right direction. 

Will I have the same Direct Support Professional (DSP)?

We strive to provide consistency by matching you with the same Direct Support Professional whenever possible. While scheduling needs, vacations, or unforeseen circumstances may occasionally require another staff member, our goal is to build long-term relationships that promote trust, consistency, and quality support.
